Requirements for all public school students The Department of Education require all public school
students to rpesent documents showing he or she has received:
One
dose of Diphteria-Tetanus-Pertussis (DTP) or Tetanus-Diphteria (Td) if the child is 7 or
older.
One
dose of the Polio vaccine (TOPV or IPV)
Kindergarten
and first grade: two doses of Measles-Mumps-Rubella (MMR), provided the second MMR was
received no longer than 30 days after the first MMR given in the child's first year of
life
For
Head Start and grades from 2 to 12: one dose of MMR or MR if the child is 7 or older
Results of a tuberculosis (TB) skin test within the last year if the child is entering
from the United States or a U.S. territory, or within six months if the child is from a
non-U.S. territory (if results are positive, child must obtain TB evaluation clearance
form from the Department of Public Health in Mangilao).
Results
of a physical exam if it is the child's first year of school, or proof that an exam has
been scheduled
A
completed Emergency Information Card, which is provided by the school.
